Description: Advocates for families to assure and protect their health and well-being. Together with our community partners, we strive to assure access to services essential to having healthy babies, growing healthy kids, and building healthy families.
Services: Comprehensive health assessments; health education, outreach, medical and dental referrals, etc. Provides diagnostic and treatment services, medical case management and physical and occupational therapy services to children/youth under 21 who have chronic, disabling or life threatening conditions. Developmental Disabilities Council provides a forum for discussion, advocacy, and program and policy planning for individuals with developmental disabilities, their families and service providers.

Eligible: Open to all women of reproductive age, children, youth and families in Alameda County.
